Last month, a semi truck accident injured two people and killed two more on I-40 in Henderson County. Tennessee Highway Patrol officers have released a report that explains how the accident occurred.

Rachel Williams, 25, was driving a car east on I-40. She switched lanes when a tractor-trailer collided with her left rear. The collision sent Williams’ car into a guardrail. The tractor-trailer, occupied by Dennis Clow, 60, and Monica Clow, 56, hit the opposite guardrail, crossed several lanes and eventually turned over before hitting another car driven by Candy Otter, 49, who suffered injuries in the crash.

Joe Wooley, 58, suffered injuries when the tractor-trailer hit his vehicle. Wooley’s car hit two other cars driven by Calen Hauber, 22, and Ronald Nance, 58.

The tractor-trailer caught on fire during the accident, and both Dennis and Monica Clow died from their injuries. Police are not sure who was driving at the time.
